Purpose: When a case is first filed, a document referred to as a filing document is often required to
submit the initial filing. If you do not have a filing document specified, the error "A filing document is required" appears on your screen.
This guide lists common filing documents and form numbers for many case types within the Wisconsin Circuit Court system. Note that this is not an all encompassing list. The filing document required for your case type and classification code may differ from the documents listed below. This guide is simply a reference list for identifying potential filing documents and should not be interpreted as legal guidance.

Uploading a filing document
Once you have prepared your filing document and have completed the additional steps to initiate your new case filing, you must select the relevant Document type to proceed with your filing. This selection appears on the Documents page of the eFiling site. If you do not select the correct Document type, you will encounter an error stating, “A filing document is required.”
